Instructions for completing the District Information form

The next step in the ordering process is to gather your District hierarchy for testing. LAS Links has a form which you will fill out to provide us with your District and School list.

By filling out this form, you will also designate a District Testing Coordinator for your district where all communications will go through as well as the main person who will be handling district processes during testing.
